Booking instructions
After checking that the name of the property, the start date and the duration of your holiday is correct please complete the remainder of this form. Required information is marked with an asterisk.

Guest details
We also require names of ALL guests, including the person making the booking.  So if there are more than one you need to click ‘add another guest' for each additional guest after Guest 1.

If any guest is under the age of 25 we require the age of that guest. It is not necessary to fill in this box for those 25 or over.

Pets
Pets are only accepted at certain properties. Please check the property description under the ‘INFORMATION AND FACILITIES' section to see whether pets are welcome and, if so, whether there is a maximum number accepted. Please also indicate size and breed of pet being brought.

What happens next?
By clicking the Send Booking Request button, you are making a provisional booking with the owner of the property. Cornish Horizons will check the details and email you back to confirm all the details, advise you of errors (if applicable) and confirm acceptance of this booking request.

You should also automatically receive an email to confirm that the Booking Request has been sent to Cornish Horizons.
